Prime Choice Home Care is proud to offer Community First Services and Supports (CFSS), Minnesota’s updated service model that replaces Traditional PCA and PCA Choice, in accordance with Minnesota Department of Human Services (MN DHS) guidelines. CFSS is designed to support individuals with disabilities or chronic illnesses to live independently in their homes and communities.
At Prime Choice Home Care, we understand that individuals may need hands-on, person-centered support to maintain health, independence, and daily comfort. Our CFSS services focus on flexibility, choice, and dignity while supporting essential daily needs. We are committed to helping individuals feel safe, supported, and empowered in their own homes.
Support with daily living activities such as bathing, dressing, grooming, mobility, and eating, based on the individual’s assessed needs and goals.
Assistance with instrumental daily activities (IADLS) including meal preparation, medication support, light housekeeping, and essential household tasks.
Services are delivered by trained support workers employed by Prime Choice Home Care and coordinated under CFSS service requirements.
A self-directed service option that allows individuals to recruit, train, and manage their own support workers, including family members or trusted individuals, while working with a DHS-approved Financial Management Services (SFM) provider.
Offers greater flexibility and control over how services are delivered, allowing individuals to tailor support to their preferences and daily routines.
Prime Choice Home Care supports CFSS participants by assisting with compliance, guidance, and administrative requirements throughout the process.
